Report Incidents, Claims, and Close Calls with Confidence

Everything you need to report an incident, submit a claim, document a close call, and find support resources.

Claims

Requires Claims Department review or follow-up.

A Claim is an incident that requires Claims Department involvement. If you believe any incident could potentially turn into a claim, select Claim when submitting your report.

Claims often involve injury, property damage, liability concerns, or situations that require investigation and follow-up.

When to Use

  • Injury to a customer, employee, or third party
  • Property damage
  • Potential liability concerns
  • Any incident that may require Claims review

Incidents

Must be documented but does not require Claims involvement. If you select Incident when reporting in the portal, the Claims Department will not take any action.

  • General Liability: Wheel Off, Loose Wheel, Fitment, Non-employee Bodily Injury
  • Auto Liability: Company Owned Vehicle Damage
  • Garage Keepers: Customer Vehicle Damages, Wheel Damages, Lost Personal Property
  • Product Liability: Tire Failure that causes damages. (Tire must be retained and shipped to manufacturer)
  • Property Damage: Company Owned Building/Property Damages
  • Security Incident/Workplace Violence (WPV): An event involving a break-in, burglary, robbery, customer or employee threat, or other non-claims situations requiring police involvement.

Close Calls

Report near misses before they become incidents.

A Close Call is an event that occurred, or nearly occurred, that could have resulted in injury, illness, or property damage but did not. Even when no harm occurs, reporting close calls helps uncover hazards, improve processes, and prevent future incidents.
